Rotura diafragmática en trauma torácico cerrado de alta energía en paciente embarazada. Caso clínico.

La rotura diafragmática traumática es un evento poco común, de difícil diagnóstico y manejo, con múltiples complicaciones asociadas, y alta morbimortalidad. Debido a lo anterior describimos un caso de una paciente de 39 años, cursando embarazo gemelar de 27 semanas, que ingresa al Servicio de Urgencia del Hospital Hernán Henríquez Aravena de Temuco, Chile, tras accidente automovilístico, refiriendo dolor intenso tóraco-dorsal derecho. Ingresa estable, ambos fetos
vivos. Se pesquisa fractura de costillas derechas, sin hemotórax.
Evoluciona con compromiso respiratorio progresivo y clínica compatible con hemotórax masivo, instalándose drenaje pleural que da salida a 1500cc de sangre; se indican corticoides para maduración pulmonar fetal y cirugía inmediata. Ecografía constata ausencia de latidos cardiofetales. Toracotomía exploradora pesquisa lesión frénica de 2 centímetros en cúpula diafragmática derecha, con sangrado activo, realizándose frenorrafia y cesárea para extracción de fetos muertos. Evoluciona favorablemente, dándose de alta 2 semanas mástarde.
